The fan and blades come as a complete unit, you then have to reverse the fan on it's shaft for it to blow through the radiator. This requires cutting a retaining slot in the fan boss to line it up with the locating pin. It takes about 10 minutes with a Dremel. Alternatively you could use the old fan if it is not seized onto the shaft.
